Incident Report
An Incident report is to be completed by an adult witness whenever an incident requiring secondary medical attention occurs (eg. injury leading to injured party going to a medical centre). The form below should be printed, completed (by hand) and be given/sent ASAP to the Community Centres office for processing – office@canbap.org.
